What Causes Diarrhea In My Minature Aussie?

I have a 3 1/2 year old Minature Aussie and in 2012 she landed in the hospital with a severe case of gastroenteritis and pancreatis. In the hospital a week she was discharged with bouts of diarrhea but soon to clear. Vet put her on a bland diet and she's still on that diet but still has bouts of diarrhea, what can we do as her care-takers to stop this from happening?

We add rice and ground beef to the dry food we give our Aussies. If you want exact measurements let me know. Teachr2014@aol.com

You can also try probiotics like Culturelle. There are other brands that are not as pricey.
